File Deletion without Deletion Action?

Hello Everyone!
I am sending a webform to an external candidate and ask him/her to upload an attachment. Sometimes I am receiving an error that the attachment is missing although the candidate has attached it in the correct format. I can see that the attachment has been there since it does not show the information "No file chosen" in the according field but shows a blank field. For all the attachments that have intentionally not been added it always gives the information "No file chosen". So the attachment/file has been there when submitting the webform but gets deleted later although none of the deletion actions have been kicking off.
Anyone knows why the file might get deleted?
