Record user that manually ended instance

When a user manually ends an instance, display who ended it and show on the instance detail screen and in the Master workflow table in a new Instance Ended By column.

    Master data table should also include a column for "Instance ended by ID" to complement the currently available Master table data column named "Instance created by ID".
